Body image issues affecting men are common yet little talked about as there is little acceptance of men complaining about their bodies. However, just like women, men also have body image concerns. Some are worried about their musculature or hair loss while others are concerned about the way their body has developed. Gynecomastia is one of those concerns. It is a common yet little-talked-about problem. It worries some men and, in many cases, they suffer in silence and shame.

Gynecomastia, commonly known as “man-boobs” involves benign growth of male breast glands and, in the great majority of cases, it is entirely benign. In one study, 65% of men aged 27 to 92 had gynecomastia as defined by breast nodule greater than 2cm. For some men, enlarged breast glands are of no concern but for others, they are very troubling.

There are multiple causes of gynecomastia and they generally involve some hormonal alterations. These alterations can be significant or, as in most cases, are very subtle and non-consequential with respect to men’s general health.

Generally speaking, gynecomastia primarily affects boys and men in three different phases of life. Up to 90% of neonates, up to 60% of adolescents, and up to 70% of men aged 50 to 69 are affected by this troubling condition. These phases of life are characterized by subtle hormonal changes. In the case of neonates, we are talking about maternal hormones. Adolescents exhibit alteration in testosterone activity and elder individuals are subject to medications as well as body conditions that can affect the levels of circulating hormones.

It is critical to properly assess men with possible gynecomastia by looking at their history as well as performing a focused physical examination. In addition, further laboratory, as well as imaging tests, may be required. Although breast cancer in men is rare, it can happen and its early detection can allow for appropriate treatment and save lives. Some of the key warning signs regarding breast cancer are asymmetry between breasts, nipple discharge as well as firm nodularity.

Although most men are not concerned about enlarged breasts, some are greatly affected by a lack of self-confidence as well as psychological distress. Fortunately, there are some treatments available. These include reassurance, oral medications as well as surgical interventions. In the majority of cases, gynecomastia resolves by itself provided that the offending agents (such as elevated hormones or medications) are identified and corrected or eliminated. In some instances, oral medications are needed and include agents designed to manipulate the hormonal imbalances.

When observation or medical treatment fails, surgical interventions may be considered. These may include direct glad excision or various forms of liposuction. Although direct surgical excision of the breast glands is the surest way to remove breast tissue, this technique can leave significant irregularities and scarring. Liposuction has been used with great success in the treatment of gynecomastia although the removal of the glandular tissues is very challenging and usually incomplete. More advanced liposuction techniques such as BodyTite offer an enhanced gynecomastia treatment modality, allowing for more predictable results.
